If the patient hasn't booked within 48 hours, the request escalates to the discharge planning coordinator for a personal outreach call, since missed post-discharge follow-ups are a major driver of avoidable readmissions.
Once booked, the patient receives standard reminder messaging leading up to the visit just like any other appointment.
